A 66% increase in quarterly revenue and record lung scan volumes have strengthened 4DMedical Limited's (ASX: 4DX) commercial momentum, but recent share-price volatility has intensified questions about its valuation.
The medical imaging technology company reported 119,672 scan analyses during the September 2026 quarter, with its software deployed across 724 sites worldwide.
However, revenue of $2.3 million remains modest against quarterly operating cash outflow of $15.2 million. An ASX price query on 2 November adds another dimension to the central question: can expanding clinical adoption generate enough revenue and cash flow to support market expectations?
Key numbers
- $2.3 million: Unaudited September-quarter operating revenue, up 66% year-on-year.
- 119,672: Quarterly scan analyses, representing 61% annual growth.
- 724 sites: Global software deployment footprint, up 77% year-on-year.
- $242.8 million: Cash at 30 September 2026.
- $15.2 million: September-quarter net operating cash outflow, with a reported funding runway of approximately 16 quarters at that rate.
Commercial adoption accelerates across healthcare networks
4DMedical's 28 October quarterly report highlighted expanding adoption across the United States, Europe and Australia.
Its Lung Density Analysis-Inspiration technology was selected for chronic obstructive pulmonary disease assessment across a major US Veterans Affairs healthcare network.
The agreement could generate approximately US$800,000 over five years, although it includes an initial one-year term followed by annual extensions.
The contract provides an institutional customer relationship, but its potential revenue contribution remains relatively small compared with operating expenditure.
Meanwhile, SimonMed completed the rollout of CT:VQ and LDAf across 125 CT-enabled imaging sites.
Stanford Medicine renewed its software agreement for another 12 months, while SimonMed secured a three-year subscription for the IQ-UIP lung fibrosis analysis platform.
These developments strengthen distribution and customer relationships.
However, growth in deployed sites partly reflects acquisitions, including contextflow, rather than entirely organic expansion. Installed systems also do not automatically translate into high utilisation or recurring revenue.
ASX price query highlights valuation concerns
On 2 November, 4DMedical responded to an ASX query concerning unusual trading activity and a sharp decline in its share price.
The company's shares had fallen from a closing price of $3.28 on 29 October to an intraday low of $2.41 on 30 October, representing a decline of approximately 26.5% between those reference prices.
The company stated that it was unaware of undisclosed market-sensitive information explaining the activity.
Management suggested increased short interest could have contributed, although this was not independently established.
The volatility highlights the sensitivity of market expectations surrounding 4DMedical's commercial growth.
Despite FY2026 operating revenue of approximately $7.1 million, its market capitalisation was around $1.5 billion in early November.
That implies approximately 211 times historical annual revenue, or 163 times the $9.2 million annualised September-quarter revenue run rate.
These illustrative equity-value-to-revenue comparisons are not formal enterprise valuation multiples, but they indicate substantial expectations for future commercial growth.
What stands out to us
4DMedical's proprietary imaging software and integration with established healthcare networks provide potential competitive advantages.
Gross margins exceeded 90% during the September quarter, suggesting attractive software economics if revenue scales.
However, financial performance remains challenging.
The company reported $15.2 million in operating cash outflows and $22.5 million in investing outflows, largely associated with the contextflow acquisition and RevealDx investment.
Customer receipts totalled approximately $1.61 million.
Although $242.8 million in cash provides financial flexibility, the reported 16-quarter runway excludes future investing and financing cash flows.
4DMedical also has a $10 million secured financing arrangement with Pro Medicus carrying 12.5% annual interest.
The agreement includes share-price-linked repayment provisions, with potential cash settlement between $12.5 million and $20 million, subject to contractual terms.
Consequently, future financial obligations extend beyond the original loan principal.
What the company is doing now
4DMedical continues expanding its diagnostic imaging portfolio through acquisitions, commercial agreements and clinical development.
In Germany, contextflow secured a deployment involving approximately 25,000 lung cancer screening procedures across 25 sites over three years.
Australian adoption is progressing through agreements involving Jones Radiology, Qscan and providers within the Integral Diagnostics network.
The company is also advancing its CLEAR clinical study, evaluating CT:VQ for suspected pulmonary embolism.
Results are anticipated during 2027.
Separately, proposed US legislation supporting a US$20 million Veterans Affairs imaging pilot remains subject to the legislative process and does not represent secured company revenue.
Insight: Can 4DMedical's Commercial Growth Support Its Valuation?
We think 4DMedical is demonstrating genuine commercial progress through expanding scan volumes, hospital relationships and gross margins above 90%. However, its $2.3 million quarterly revenue remains modest against $15.2 million in operating cash outflow. Substantial cash reserves provide development flexibility, but the valuation still depends on considerable future revenue growth. Recent share-price volatility reinforces the gap between commercial expectations and demonstrated financial performance. Our assessment remains constructive on technology adoption but cautious on valuation until customer utilisation, recurring revenue and cash conversion improve materially.
