Pro Medicus Limited (ASX: PME) has secured $67 million in new US healthcare contracts within three days, extending its expansion into cloud-based medical imaging and strengthening its long-term revenue pipeline.
Combined with an August agreement, the latest wins bring newly announced contracts to $92 million since the start of FY27. The opportunity is substantial, but can Pro Medicus convert its expanding customer base into sustained earnings growth while maintaining industry-leading profitability?
Key numbers
- $92 million: Combined value of three seven-year contracts announced between August and October 2026.
- $261.7 million: FY26 revenue, increasing 22.9% year-on-year.
- $144.7 million: FY26 underlying net profit, up 24.1%.
- 74.9%: FY26 underlying EBIT margin, improving 90 basis points.
- $252.3 million: Cash and other current financial assets at 30 June 2026, with no debt.
Two October contracts strengthen the US expansion story
On 21 October 2026, Pro Medicus announced a seven-year, $43 million contract with New Jersey healthcare provider Virtua Health.
The agreement covers its Visage 7 Viewer, Workflow, Open Archive and Cardiology solutions, delivered through a unified cloud-based platform.
Deployment is targeted for late in the second quarter or early in the third quarter of calendar 2027.
Two days earlier, the company secured a seven-year, $24 million agreement with Billings Clinic–Logan Health, covering 25 hospitals and more than 1,000 beds.
That deployment is targeted for the first quarter of 2027.
These contracts follow a $25 million agreement with Valley Health announced in August.
What makes these developments commercially important is their transaction-based licensing structure, which provides potential revenue upside as imaging volumes increase.
The broader product scope also demonstrates Pro Medicus' ability to sell integrated enterprise imaging solutions rather than individual software applications.
However, the combined $92 million represents contracted value across seven-year agreements, not immediately recognisable revenue.
Why FY26 earnings reveal a stronger underlying business
Pro Medicus entered FY27 following another period of profitable expansion.
Revenue increased from $213 million in FY25 to $261.7 million in FY26, while underlying EBIT reached $196.1 million, growing 24.4%.
Its 74.9% underlying EBIT margin demonstrates considerable operating leverage. Software licensing allows additional revenue to contribute substantially to earnings without proportionate increases in operating expenditure.
The company also secured $407 million in new contracts during FY26 and renewed six agreements worth $141 million.
Minimum contracted revenue over the following five years reached approximately $1.3 billion, providing considerable forward visibility.
One important distinction concerns statutory profitability.
Reported FY26 net profit reached $265.3 million, increasing 130.3%, but this included substantial unrealised fair-value gains associated with financial assets.
Underlying net profit of $144.7 million provides a more useful indication of recurring business performance.
Operating cash flow reached $127.5 million, compared with $111.3 million in FY25, representing approximately 14.5% growth.
Although cash generation remained strong, its slower growth relative to revenue makes cash conversion and working capital important measures to monitor as the business expands.
Can the balance sheet support another expansion phase?
Pro Medicus' financial position remains a competitive advantage.
At 30 June 2026, the company held $146.5 million in cash and cash equivalents, with cash and other current financial assets totalling $252.3 million. It also reported no debt.
This provides considerable financial flexibility to support product development, customer implementations and strategic opportunities.
New digital pathology and AI-optimised reporting capabilities could expand revenue opportunities within existing customer relationships.
The Trinity implementation, substantially completed during FY26, is also positioned to contribute across FY27.
Nevertheless, valuation remains an important consideration.
Pro Medicus' exceptional underlying earnings margins and recurring revenue visibility support a premium business assessment. However, whether that premium represents attractive value depends on sustained earnings growth and the valuation multiple applied to normalised profits.
Cloud migration delays, competitive pressure, currency movements and slower healthcare spending could challenge growth expectations.
Insight: Contract conversion matters more than contract announcements
What stands out to us is Pro Medicus' combination of contracted revenue visibility and unusually strong operating margins. Its approximately $1.3 billion five-year minimum contracted revenue pipeline provides a foundation for continued growth, while transaction-based licensing creates potential upside beyond minimum commitments. However, the market may be underestimating the time required to implement complex healthcare systems. We see the strongest evidence of further value creation emerging when new deployments translate into recurring revenue without weakening margins. The next test is FY27 revenue growth and underlying EBIT margin.
